Insurance Terminology

Deductible
The amount you pay for covered health care services before your insurance plan starts to pay.

Coinsurance
The percentage of costs of a covered health care service you pay (20%, for example) after you've paid your deductible.

Copayment
The amount you pay to a healthcare provider at the time you receive services. You may have to pay a copay for each covered visit to your doctor, depending on your plan. Not all plans have a copay.

Explanation of Benefits
The insurance company’s written explanation regarding a claim, showing what they paid and what the patient must pay.

Visits Maximum
This is the maximum number of visits your insurance company will pay. However, this is not a guarantee. Often, an insurance company will state a high number, or even unlimited visits, but will deny payment after review of medical notes if they don’t consider treatment to be justified.

Active Care
A form of treatment that expects an improvement of the diagnosed condition. Insurance will cover this form of care.

Supportive / Maintenance / Performance Care
A type of care that helps prevent regression of a current condition or allows the body to functioning optimally without the expectation of improving a specific condition. Insurance does not cover this form of care but we offer affordable self pay rates.
